You may fill these out in advance, but the completed forms must be submitted in person to the Administration office and cannot be submitted online. Certain forms may not be submitted without the required documentation needed for initial student registration.
Please bring the following required information when registering students:
- Copy of birth certificate
- Copy of social security card
- Shot record
- Proof of enrollment at previous school (withdrawal paper or final report card)
- Proof of residency (utility bill, etc...)
